Primary Total Hip Arthroplasty Using Multilock Femoral Stem in Non-traumatic Osteonecrosis of the Femoral Head - A Comparison of under 50 and over 50 years old -

Abstract
Purpose: To analyze the clinical and radiological results of total hip arthroplasty, using a Multilock femoral stem, between patients under and over 50 years old, with osteonecrosis of the femoral head.

Materials and Methods: Between Jan. 1992 and Feb. 1998, the results of 43 cases under 50 years old (group 1) and 47 cases over 50 years old (group 2), in whom a total hip arthroplasty had been performed with a Multilock stem, were retrospectively reviewed. All had Ficat stage III or IV osteonecrosis of the femoral head. The mean follow-up periods were 7.9 and 7.8 years in the group 1 and 2, respectively.

Results: The final mean Harris hip scores were 94.7 and 95.0 in the group 1 and 2, respectively. Femoral osteolysis was detected in 9 and 10 cases in the group 1 and 2, respectively. Acetabular osteolysis was detected in 9 and 8 cases in the group 1 and 2, respectively. The average linear wear rates of the polyethylene liner were 0.138 mm/yr and 0.131 mm/yr in the group 1 and 2, respectively. Bony ingrowths were observed in 43(100%) and 45(96%) cases in the group 1 and 2, respectively.

Conclusion: The primary total hip arthroplasty with a Multilock femoral stem showed satisfactory results in patients with osteonecrosis of the femoral head. However, osteolysis remains of concern.
